Time: During the tryouts
All forms must be completed, fee paid in full and you must have all proofs of residency with original birth certificate for the player and parent/guardian photo ID.
Acceptable proofs of Residency are Three of the Following:
- Driver’s License
- Voter’s Registration
- School Records
- Welfare/Child Records
- Federal Records
- State Records
- Local (municipal) records
- Support payment records
- Homeowner or tenant records
- Insurance documents
- Medical records
- Military records
- Vehicle records
- Employment records
- Internet, cable, or satellite records (ie.Comcast/DirecTv)
- Utility bills (ie gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
- Financial (loan, credit, investment, etc) records
Please note that all proofs of residence need to be dated September 1, 2011 or after!!
