"Ack! Apply for Life Insurance? I'd rather go to the DMV!!"

Many people worry that applying for life insurance is a long, complicated and intrusive affair. (Kind of like Health Insurance...) Nothing could be further from the truth!

Here's a look at the questionnaire my agency uses to get the information we need to set up a life insurance policy: 

BASIC LIFE INSURANCE RATING INFORMATION:

General Information

·         Name

·         Gender

·         Date of Birth

·         Place of Birth

·         Height and Weight Listed on Drivers License

·         Height and Weight Currently

·         Address

·         Phone Number

·         Drivers License Number and State

Personal Questions:

·         Do you have any life insurance now?

·         Occupation?

·         Are you a US Citizen?

·         Beneficiaries: (Who do you want to get the money...)

·         Do you take part in any “dangerous” activities? (Pilot/Scuba/Skydiving/etc)

·         In the last 2 years, have you been diagnosed with/treated for cancer, tumor, stroke, alcoholism, or heart/liver/kidney problems?

·         Have you had any other major health problems/disorders in the last 7 years?

·         Do you have HIV/AIDS?

·         Have you been hospitalized for more than 24 hours in the last year?

·         Do you have any surgeries currently scheduled?

·         Have you used tobacco in the last year?

·         Have you used illegal drugs in the last year?

·         Have you been convicted of a felony in the last year?

·         Do you prefer chunky, or smooth peanut butter? (just kidding…)

That's it...  Not too bad, right?  This is pretty much all we need to get you covered.  If you don't want a quick physical exam, or have health problems or family medical history issues, there are also policies available that don't care about any of that!
